When you complete the intake phone call, we will invite you to our client portal via Therapy Notes and send you the required documents through the Therapy Notes portal.

               If for some reason you are unable to access the
               client portal, please fill out the following forms linked on this page or reach out to our Intake Specialist or your
               therapist for additional support.

Please note all pages must be filled out prior to your first appointment. This helps ensure your health & safety, and helps your therapist be more informed at your first meeting. Thank you for your help!
